Insurance recipients eligible for MCG benefits in 2023, will be provided with 1095 forms no later than early March 15, 2024. The 1095 form reports information about healthcare coverage as required by the Affordable Care Act. This form is NOT needed to file individual tax returns, but should be kept with 2023 tax documentation. It does NOT replace a W-2.

The 1095-C form will be mailed to every individual who was eligible for benefits. Covered dependents will not receive their own 1095 form.

For 2023 Kaiser 1095 forms will be available online through the member's portal at Kaiser Permanente. Copies of the 1095's will be mailed to Kaiser participants by March 15, 2024. If it is not received after that date, please contact member services at 877.514.5114. All forms should be available by March 15, 2024. Montgomery County will mail CareFirst and United Healthcare members.

These forms will be mailed to the home address on record. Please make sure OHR has a current up to date address in Employee Self-Service.

Be aware of possible phishing scams regarding 1095 forms. As stated above, 1095 forms will be mailed to recipients. Do not click on any links that you may receive via text or email related to your 1095-C form.
