Asian American Health Initiative (AAHI) Community Outreach and Health Education

AAHI provides outreach and education designed to increase awareness of health and wellness concerns pertinent to Asian Americans, including chronic health conditions, social needs, senior wellness, and mental health concerns.

AAHI disseminates relevant health information to the Asian American community at community-based organizations, faith-based organizations, cultural events, health fairs, and small businesses.

AAHI has culturally and linguistically tailored health education materials, available in English, Chinese, Korean, Vietnamese, and Hindi on the web site in URL 2, as well as available to order in print for pickup on the website in URL 3.

In addition to education, AAHI also provides navigation support for health care and social services throughout Montgomery County to eligible residents through the Patient Navigator Program (PNP). Details of the PNP program are available separately. Please note that the Patient Navigator Program (PNP) is contracted to an external entity ? Cross Cultural Infotech.

AAHI is not a clinic, nor is it a hands-on health care provider.
