Dependent children turning 26-years and losing eligibility of coverage as a dependent child, may be required to present a Termination of Insurance Coverage letter to obtain coverage thru their employer or private insurance plan. This letter may be obtained from OHR's Health Insurance Team by sending a service request via MC311.
