Unable to Locate Information Packet Received at MPDU Orientation and Would Like to Receive a Copy

Send an email to the MPDU Program at requesting another copy of the packet. State in the email the name and current address of the person that attended the MPDU Orientation Session, and the date that the household attended the MPDU Orientation Session. The MPDU staff will mail out a replacement packet.
