Targeted Case Management services for low income clients with a serious mental illness are very limited except for individuals that are in existing mental health treatment programs; discharged from psychiatric hospitals, or homeless. These are provided by providers in the community, not Montgomery Co HHS. Current providers are Community Residences, Inc. and PDG.
