Completion of this form WILL NOT replace the need to request any necessary repair permits or file any associated insurance claims nor will it guarantee individual eligibility for federal, state, or local reimbursement. The County will not be responsible for fixing or cleaning up damage on private property. This assessment is entirely focused on assessing County eligibility for FEMA Individual Assistance. In cases where severe damage is identified through our screening process, information will be sent along to the Department of Permitting Services (DPS) to support follow-up with those impacted residents.

Should residents find damage that may pose an imminent risk to life or safety, they should immediately dial 9-1-1. In addition, when photographing damage, residents should not put themselves in potentially dangerous situations including climbing on debris, approaching downed wires, or entering compromised structures.
