To report a complaint resulting from service provided by Fire Rescue staff, please click on the Create Service Request button below. Make sure to provide the requestor's full name; phone number; address; email address and a complete description of the issue including date and time of the incident; location of incident; names of Montgomery County Fire and Rescue (MCFRS) staff; and vehicle numbers of the staff. A representative from MCFRS will respond to the Service Request and investigate the reported issue.
