A Medicaid Transportation Program application may be faxed to 240.777.5891 or emailed to Medicaidtransportation@montgomerycountymd.gov or mailed to the Medicaid Transportation Office at 101 Monroe Street, 5th Floor, Rockville, MD 20850. Hand delivery is also acceptable.
