Group or Nursing Homes, Assisted Living or Domiciliary Care Facilities

To operate a group or nursing home, domiciliary or assisted living facility, a provider is required to have BOTH a State license and a Montgomery County license. See links below for more information and how to obtain both licenses. Applications for a nursing home license can also be obtained at the Licensure and Regulatory Office, 2425 Reedie Drive, 9th Floor, Wheaton, MD. 20902, between the hours of 8:00 AM and 4:00 PM. Monday through Friday. It takes approximately 30 days to issue a license.
