To be removed from the voter roll, a request must be submitted to the Board of Elections (BOE) There is a Cancellation Form on our website, or a written request may be submitted. Include the voter's complete name, address, date of birth and must indicate the desire to be removed. Sign and date the request and mail, fax or send via email (include attachment with signature) to the BOE. The fax number is: 240.77.8600. The BOE is located at: 18753, Suite 210 N. Frederick Av., Gaithersburg, MD. 20879. Office hours are Monday through Friday, 8:30 AM - 5:00 PM, except holidays. Extended hours are offered during election season.
