Group Homes Nursing Homes Hospitals or Domiciliary Care Facilities

To operate a group home, nursing home, hospital, or domiciliary care facility, a provider is required to have BOTH a State and a Montgomery County license. See links below for more information and how to obtain both licenses. Applications for the health care facilities listed can also be obtained at the Licensure and Regulatory Office, 2425 Reedie Drive, 9th Floor, Wheaton, MD. 20902, between the hours of 8:00 AM and 4:00 PM. Monday through Friday.

For complaints about any of the listed health care facilities, the Office of Health Care Quality (OHCQ), MD Dept of Health may be contacted at 1.877.402.8218 or online. For information about senior health care facilities and resident advocacy services in licensed senior group homes, nursing homes or domiciliary care facilities, individuals should contact the Ombudsman program at: 240.777.3369.
