Name Change (Current or Former Employees Only)

Employees and former employees who are not retirees must email the Office of Human Resources (OHR) Records Management team at [email protected] to request a Name Change Form. Complete and sign the form, then return it by email along with a copy of your updated driver?s license, Social Security card, or court order as proof of the legal name change.
