For women who have APPLIED for the program and are not yet eligible (application is still in progress) or someone who wants to apply, please check the Health Care Eligibility Unit information for information on how to check status, and/or make an application for the program.
The Germantown Health Center is located in at the Up-County Government Center at 12900 Middlebrook Road, Germantown, 20874. Hours are: 8:00 AM - 4:00 PM, Monday through Friday. Offerings include: Case Management services to eligible pregnant women and their children up to age one year who are enrolled; assistance to pregnant teens and Infants at risk referred by hospitals and or medical providers.
Case management services include home visits and telephone calls by Nurses and Community Service Aides to offer health and social assessments, prenatal education, newborn and infant care education, support navigating the health care system, family planning and education, and linkage to medical providers.
Limited Oral Health (Dental care) services are covered and provided by the County?s Dental Program. Dental Appointments are offered as part of enrollment in the MPP Program. General dental care services are provided to certain pregnant women who are enrolled in the DHHS MPP and to children who are on Medicaid. The Center also provides routine childhood Immunizations, offered to children up to age 19 who have active Maryland Medical Assistance or are uninsured, or whose medical provider does cover/offer immunizations.
