A Voter Notification Card is NOT required in order to vote. There is a $1.00 service charge to replace a lost or stolen Voter Notification Card, if a replacement is desired. To obtain a replacement card, voters may visit the Board of Elections Office at: 18753, Suite 210, North Frederick Road, Gaithersburg, MD. 20879. Hours of operation are Monday through Friday, 8:30 AM - 5:00 PM. Voters may also mail a request for a replacement card to the office. Be sure to include name, address, voter's date of birth, date and signature. There is a $1.00 service charge to replace a (i.e. lost or stolen) Voter Notification Card, if a replacement is desired sooner than later. One may also be requested online via the Voter Lookup portal at no charge .
