Offerings include: Case Management services to eligible pregnant women and their children up to age one year enrolled in the Maternity Partnership Program (MPP); assistance to pregnant teens and Infants at risk who are referred from hospitals and or medical providers.

Case management services include home visits and telephone calls by Nurses and Community Service Aides to offer health and social assessments, prenatal education, newborn and infant care education, support navigating the health care system, family planning and education, and linkage to medical providers. Limited general dental care services are provided to pregnant women who are enrolled in the DHHS Maternity Partnership Program and to children who are on Medicaid. Assistance is also provided to clients in accessing health care, economic and self-sufficiency services and programs.
