African American Health Program AAHP

The African American Health Program (AAHP) addresses health disparities that are disproportionately affecting African Americans, Africans and individuals of Caribbean descent in the areas of: diabetes; HIV/AIDS; cardiovascular disease; maternal and child health; infant mortality/babies born healthy (S.M.I.L.E.S); oral and dental health; and cancer education. Although services are targeted to African Americans, Africans and people of Caribbean descent, they are open to all. Services provided include nurse case management for pregnant women 28 weeks or less who are at risk for an adverse outcome; referrals for oral and dental health screenings; nutrition and fitness activities; diabetes self-management education and prevention; hypertension screening (blood pressure screenings) and education; HIV counseling and testing and health educational sessions to communities where African American, Africans, Caribbeans and individuals who are of Caribbean descent are living in Montgomery County, MD.
