Name Change (Current or Former Employees Only)

Employees (or former employees) with a legal name change request must contact the Records Management team via email at Records.OHR@montgomerycountymd.gov and request a "Name Change Form. The form must be completed, signed, and emailed to the Records Management team with a copy of an updated driver's license, social security card or Court Order.
