To schedule a disability counseling appointment with a Retirement Analyst, please provide the following information: Employee's name, street address, and daytime telephone number. Please indicate whether the employee is in the Employee Retirement System (ERS), Retirement Savings (RSP) or Guaranteed Retirement Income Plan (GRIP).

Bring completed "Application for Disability Benefits" to the appointment. A member of the Retirement Team will call to answer any questions and setup the appointment.
