A W-9 for Montgomery County Government is for Organizations who are County's Customers and need to have the County's name and tax ID information.
To obtain a blank W-9 Form: "Request for Taxpayer Identification Number and Certification" click on the link below. Send the completed and signed form to Accounts Payable. Fax to 240.777.8852, regular mail to, Montgomery County Finance, Accounts Payable, 101 Monroe Street, 8th Floor, Rockville, MD 20850 or email to APSupplierMaintenance.Finance@MontgomeryCountyMD.gov. Once the W-9 is received the data will be matched to the Internal Revenue Service (IRS) TIN matching records.
