Employees and participating agency employees who are eligible for medical insurance will receive 1095-C forms no later than March 15 each year. This form is NOT needed to file individual tax returns but should be kept with your tax records.
The 1095-C form will be mailed to every individual who is eligible for benefits. Covered dependents will not receive their own 1095 form.
Kaiser 1095 forms will be available online through the member's portal at Kaiser Permanente. Copies of the 1095's will be mailed to Kaiser participants by March 15. If it is not received by that date, please contact member services at 877.514.5114.
Montgomery County will mail CareFirst and United Healthcare members. These forms will be mailed to the home address on record. Please make sure OHR has your current address in Employee Self-Service.
Be aware of possible phishing scams regarding 1095 forms. As stated above, 1095 forms will be mailed to recipients. Do not click on any links that you may receive via text or email related to your 1095-C form.
