Employee - Form 1095-B - Kaiser Participants

Employees who have Kaiser insurance will receive the 1095 directly from Kaiser. 1095 forms will be available online through the member's Kaiser Permanente account. To request a paper copy, please contact member services at 877.514.5114. All forms should be available by February 15 each year. Employees do not need to wait to receive these forms to file their tax returns but a copy should be retained with tax records. This form includes the employee's name, employee's dependents' names, the months of the year the employee was eligible for health insurance coverage and the cost of the least expensive monthly premium the employee could have paid under the insurance plan.

This form will be available to members who are registered with kp.org. To create an account, please visit the link below. Once registered, click "My coverage and costs", then "Health plan documents". The link is on the far right hand side of the page. If Kaiser participants do not receive a 1095-B by March 7 or do not have computer access, please contact Kaiser at 1.844.477.0450.
