Assistance Completing Department of Health and Human Services Forms or Applications

If customer is working with a case manager or already enrolled in a service or program, please contact the case manager or program manager involved for assistance. If there is no case manager or the individual in question is not enrolled in a program, please contact the program personnel for the particular service the form is needed. If not applicable, call Aging and Disability Resource Unit at 240-777-3000. Hours are Monday and Friday from 8:30 AM to 5:00 PM and Tuesday, Wednesday and Thursday from 8:30 AM to 7:30 PM.
