Prospective taxi drivers must first obtain LiveScan fingerprint application and a taxi application from the Taxi Regulation Office in the Executive Office Building, 101 Monroe Street, 5th Floor, Rockville, Maryland 20850. The applicant must complete the applications; complete a physical examination with tuberculosis test or X-ray; obtain a signature on the application from a doctor's office; obtain a certified driving records from any state in which they have driven for the past three (3) years; and provide four passport-size photos (three front view, one side view). The applicant should then take the LiveScan fingerprint application Criminal Justice Information System (CJIS) (directions provided with application) and obtain a receipt that fingerprinting has been completed. When the Taxicab ID application is completed, these items must be submitted with the completed application. The Taxicab Regulation office will not charge a fee to submit a new taxicab driver ID application, and only charges a $50.00 new ID issuance fee (Cash or Check only). Montgomery County no longer tests applicants applying for a Taxicab ID Card. A VALID DRIVER'S LICENSE IS REQUIRED TO RECEIVE LIVESCAN FINGERPRINT APPLICATION.
