Patient Navigator Program to Assist the Low-Income Asian Community

The Patient Navigator Program provides two main services: (1) Multi-lingual Health Information and Referral Telephone Line; (2) Trained Multi-Lingual Medical Interpreters.

The services provided by the Multi-lingual telephone line include, but are not limited to: helping callers navigate Montgomery County health resources; helping callers gain access to existing disease prevention screening programs; providing information and referrals to County health services; assisting with interpretation needs over the phone; disseminating information about health service options available to eligible community members. The program is not a clinic - it is for people looking for health care services. Patient Navigator Program staff can also provide medical interpretation services for eligible clients who are receiving services at Montgomery Cares clinics and other Department of Health and Human Services' programs in Montgomery County.
