ID Card for Medical, Dental, Prescription or Vision Insurance Plan

If an employee or retiree needs a replacement Insurance ID card for themselves or a dependent, or if the information on the card is incorrect, they should contact the insurance provider directly or visit the carriers' website to print a copy of the ID card. See the "Health Insurance Contacts" list.

However, if the update is due to a legal name change, this cannot be completed through the insurance provider. The name change must be reported to MCG, after which it will be sent to the carriers, and a new ID card will be issued automatically.
