Employees who are (1) trying to enroll a new dependent who is also a County employee OR (2) have a dependent aging out between now and the end of the year OR (3) needing to re-enroll a dependent who was removed during the 2021 Bolton recertification process cannot use the Self-Service Benefits system to make their elections. If any of these situations apply, you may use the 2027 Active Open Enrollment Election Form linked below. You can submit this fillable form online, or you can print it and fill it out as a paper form and then submit the form and any required dependent documentation via one of the following methods:

All forms and required documentation must be received by OHR by 5:00 PM on October 23, 2026. When enrolling new dependents, the appropriate documentation is required. Dependents who already had active coverage prior to 2027 do not need to submit additional documentation.
