Online users who would like to report a possible violation by a retailer or who have additional questions and would like to be contacted by a departmental staff member, may submit a request by clicking on the Create Service Request button below. Please complete all of the fields in the Contact Information section, provide the retailer's property address in the Service/Incident Address information section and provide a brief description of the request including the date of the incident, the business name, and any other pertinent details in the Brief Description field.

For inquiries requiring the response of an investigator from the Department of Environmental Protection, residents will typically be contacted within 3 days although resolution of the issue may take longer depending on the circumstances of the case.
