Accessing Health and Human Services Client Records or Files
To review or obtain copies of client or customer records, contact the Health and Human Services (HHS) program that provided the service. Program staff will provide a form to submit the request in writing. If there are additional questions about accessing information regarding client records, or if an attempt to obtain information in records was unsatisfactory, contact the DHHS Privacy Official by sending a service request. Online users may create a Service Request by clicking on the appropriate button below. Please provide the customer's name, telephone number and email address, along with a brief description of the problem or issue including information on the service or program.
