Obtain a Birth Certificate
At this time, Montgomery County is not authorized to issue a Maryland Certificate of Birth. To obtain a birth certificate for Montgomery County residents, the following options are available: Health Departments: Frederick County, (301) 600-1029; Howard County, (410) 313-6300; Prince Georges, (301) 324-2940. Other options: Maryland State Division of Vital Records: 410-764-3170 or 410-764-3181; Internet request, will receive in 5-7 business days (see link below); Mail/Fax Request: Complete the Maryland application (see link below) and send to: Division of Vital Records, Maryland State Division of Vital Records; P.O. BOX 68760, Baltimore, Maryland 21215. Along with the completed application include; a copy of requestor's un-expired government issued photo ID (such as driver's license, passport); a self-addressed-stamped envelope and $24.00 per certificate. Check or money order payable to: Division of Vital Records, OR include credit card information if paying by credit card. A reply is normally received in approximately 10 business days. Montgomery County no longer issues statement of age cards.
