Employee - Status Changes to Insurance Coverage

Changes to insurance coverage can be made for qualified status changes occurring during the year. Examples of qualified status changes are: changes to marital status, employment, dependent eligibility, births, adoptions, deaths and loss, gaining other insurance coverage, or court orders. Requirements for changing insurance coverage information reside in the "Changing Your Health Insurance Elections" document and it can be viewed by clicking on the link below.

Proof of life status change along with a completed "Health and Life Insurance Elections Form" form are required within 60 days of the status change. Documentation should be submitted to the Health Insurance Team, Executive Office Building, 101 Monroe Street, 7th Floor, Rockville, MD 20850 or faxed to 240.777.5131. If insurance change requests are not submitted within 60 days of the life event, changes must wait until the next enrollment period.
