WISH Program - Wellness and Independence for Seniors at Home

WISH is a free program that connects seniors to community based health care and other support services to promote wellness independence at home. The goal of the WISH program is to reduce avoidable hospital use by connecting people age 65 or older and living independently in the community, to the support and services they need before their health declines.

Through a confidential health survey and an in-person coaching session a WISH Health Coach will work to empower seniors by developing a personalized plan to address specific needs, providing guidance on navigating community and other resources, and arranging services that may prevent unnecessary hospital contact. Qualifying seniors are age 65 or older, have Medicare, and live in a house or apartment in one of the following zip codes:
