Employee - Optional Life Insurance Evidence of Insurability

There are no medical underwriting requirements for Optional Life Insurance coverage up to the lesser of four times the employee's basic annual earnings, or $400,000 (the maximum coverage that will be guaranteed without underwriting) for the 2016 Open Enrollment period.

Optional Life Insurance coverage of five to eight times the employee's basic annual earnings requires that an Evidence of Insurability (EOI) form be approved by Minnesota Life (life insurance carrier for 2016). The Open Enrollment system automatically asks for this information regardless of employee's election, but employees are only required to submit an EOI form if they receive it from Minnesota Life, after Open enrollment ends. The EOI form must be completed and submitted to Minnesota Life within 30 days of receipt.
