To operate a nursing home, the provider is required to have BOTH a State license and a Montgomery County license. please click on the appropriate link below for complaints; complaints may also be made by phone: 1.877.402.8218 or by contacting the Nursing Home Ombudsman's office at: 240.777.3369 (this office does not make recommendations to Nursing Homes). Applications for a nursing home license can be obtained at the Licensure and Regulatory Office, 255 Rockville Pike, 1st Floor, Suite 100, Rockville, MD 20850 between the hours of 8:00 AM and 4:00 PM. Monday through Friday. It takes approximately 30 days to issue a license.
