Death Certificates

The Office of Vital Records, located at 2000 Dennis Avenue in Silver Spring, 20902, can only issue a Maryland Certificate of Death when the event occurred after January 1, 2015. Applicants may obtain certified documents when it is satisfied that the applicant has a direct or tangible interest in the content of the record.

The cost per application is $20 for each document, plus $12 administrative fee. An applicant must present an unexpired Government issued photo ID. Payment options include: check, cash or money order payable to: Montgomery County. The Office also has customer walk-in hours from 8:00 AM to 11:00 AM Monday through Friday (except major holidays). To ensure staff availability, one is encouraged to call 240.777.1757 prior to going in.

For certificates of death where the death occurred prior to January, 2015, documents must be obtained through the Maryland State Division of Vital Records, 6764B Reisterstown Road, Baltimore, MD. 21215. Walk in Office hours are 8:00 AM - 4:00 PM Monday through Friday (except major holidays). See link below for additional information.
