1095-A Tax Form for Affordable Health Care

The Maryland Health Benefits Exchange (Maryland Health Connection) has begun mailing 1095-A forms, to individuals and families who are enrolled in health insurance coverage through the health insurance market place (also known as Affordable Health Care). Applicants who had Qualified Health Plan (QHP, or private health plan coverage) at any point during 2016 will receive a form by U. S. mail. If there are any questions/errors regarding the 1095-A, the state Consolidated Services Center must be contacted at: 1.855.642.8572. The state call center has identified a group of employees to address any 1095-A questions or concerns.

Applicants in the process of applying are not impacted. The 1095-A forms are only for those who had QHP coverage in 2016.

Residents with pending/incomplete applications must finish the application process and enroll in a QHP plan before January 31, 2017. In those cases, a 1095-A will be sent in 2018, to assist with income tax submissions for calendar year 2017.
