Family Medical Leave Application Process

Beginning Monday, January 11, 2016, County employees can submit and manage requests for leave under the Family and Medical Leave Act of 1993 (FMLA) through Employee Self-Service (ESS). Employees can submit FMLA leave requests, check the status of pending requests, submit medical certifications and other applicable paperwork online, from any computer with internet access.

If submitting a hard copy, the FMLA forms can be found by clicking on the link below labeled "FMLA Employee Packet".

All forms and information must be submitted within 15 days of request or a FMLA qualifying event. If the employee has further questions, please call the FMLA Helpdesk at 240.777.5137 or email fmla.information@montgomerycountymd.gov
