Maternity Partnership Program, Case Management and Education - Germantown

Community Health Nurse Case Management services are offered to eligible pregnant women and their children up to age one year enrolled in the Maternity Partnership Program (MPP); pregnant teen and infants at risk referred from hospitals and/or medical providers. Case management services include home visits and telephone calls by Nurses and Community Services Aides to offer health and social assessments, pre-natal education, newborn and infant care education, support navigating the health care system, family planning and education, and linkage to medical providers. Limited Oral Health (Dental Cares) services are covered and provided by the County?s Dental Program. Dental appointments are offered as part of enrollment in the MPP Program. Please note: eligibility for this program begins in the Office of Eligibility and Support Services (OESS), in the Department of Health and Human Services.
