Retiree - Submitting Open Enrollment Forms and Documentation

If the "Group Insurance Election Form" is used to make Open Enrollment changes, it can be mailed or faxed. If making changes online, do not complete or mail the Election Form.

All forms and required documentation can be either faxed to 240.777.5131 or mailed to the Office of Human Resources, 101 Monroe Street, 7th floor, Executive Office Building, Rockville, MD 20850.
