Reporting a Breach or Lost or Stolen Health and Human Services Client Information

To report a breach or lost or stolen DHHS client information, online users may submit a request by clicking on the Create Service Request button below. Please complete all of the fields in the Contact Information section and provide a brief description of the request in the Brief Description field.

If the incident involves electronic information, refer customer to the County Security Official at 240.777.2955. If the Security Official can't be reached, County employees can also call the IT Help Desk at 7.2828. Explain that assistance is needed to address a security incident involving HHS client information.
