Employee - Update Emergency Contact Information

Current employees may change emergency contact information by either using ePortal Self Service or by printing the Personal Data Form (PDF) referenced in the link below. The completed and signed form can be dropped off or mailed to OHR Records Management, 101 Monroe Street, 12th Floor, Rockville, MD 20850 or faxed to 240.777.5130. PDF forms are also available in OHR, EOB 12th Floor.
