Nursing Home, Domiciliary Care, Assisted Living Facility

A Domiciliary/Assisted living facility is a residential or facility-based program for 17 or more individuals, providing housing, supervision, assistance and health-related services to individuals who are unable to perform or who need assistance in performing activities of daily living.

To operate a Domiciliary/Assisted living facility the provider is required to have BOTH a Montgomery County license and a State license. To obtain information about applying for a Montgomery County Domiciliary/Assisted living license, go on line at site under URL 1. To obtain information about applying for a State Assisted living license, go online at URL 2. Click on assisted living.

Complaints about a Domiciliary/Assisted living facility can be made online at the site in URL 2. Click on assisted living then click on complaint form. Complaints are also taken by phone, toll free: 877.402.8221 to file your complaint. For recommendations, contact the Nursing Home Ombudsman's office at: 240-777-3369. One may also apply at 255 Rockville Pike, 1st Floor, Suite 100, Rockville, MD., 20850 between the hours of 8 AM and 4 PM. Monday through Friday.
