Employee - Health Insurance Carrier Requires Current Information

Employees who have changed their address or other demographic information with the County but their health insurance plan has incorrect or contains old information, may submit a request by clicking on the Create Service Request button below. Please complete all of the fields in the Contact Information section and provide a brief description of the request, including the name of the health insurance plan that needs the updated information, in the Brief Description field.
