Medical Transportation Program Application Submission
Medicaid Transportation Program application may not be faxed. The completed original application must be mailed to the office. Only original applications are processed. The address is Medicaid Transportation Office at 101 Monroe Street, 5th Floor, Rockville, MD 20850. Hand delivery is acceptable.
